This form is for Certified LeadYoung & LeadWell Trainers only, using LeadWell curriculum (ages 14-18). (If you're ordering LeadNow & LeadWell materials, complete the different forms, but let us know of the 2 orders so we can ship them together.)

List 1st Module Students/Leaders: first & last name, application date, #1 SIS score, #2 SIS score (i.e. Jesse Nelson, AM, 5/13/11, 3.88, 4.04).  Please start a new line for each participant.  These leaders will receive the workbook, concept cards, and completion pin, along with 1st time materials: LeadYoung book & card holder.

 

List 2nd Module Students/Leaders: They'll receive a LeadWell portfolio & pen along with their regular workbook, completion pin and concept cards. 

NOT YET AVAILABLE: List 3rd Module Student/Leaders: They'll receive a v-neck, LeadWell t-shirt, along with their workbook, completion pin, and concept cards.  Be sure to clarify the t-shirt size and whether it is male or female.  Sizes include Adult Sm, Med, Lg, XL, XXL, XXXL.

 

NOT YET AVAILABLE: List 4th Module Student/Leaders: They'll receive a 6" chenille LeadWell letter "L" and a framed certificate, so please be sure spelling is proofed.  This is in addition to the workbook, completion pin, and concept cards.

 

Koach cues (How many sets do you need?)  

 Misc.  Please provide us with any special instructions or re-orders for books, card holders, Koach shirts, etc.  We'll let you know if there are any extra charges before shipping the order (i.e polo shirt, $35).

 

Be sure to push the "Submit Form" button, after which you should see a confirmation screen.  We'll e-mail an invoice to you as soon as we ship the order.  We're a non-profit, so prompt payment is appreciated or at least within 30 days.  We really do what to serve you with excellence, but we're new and learning, so let us know how we can improve.  Thanks again for investing in young leaders!
